I,
_____________________________________________, wish to receive my Confirmation in the Fall
of 2011 at SMS. In preparation for receiving this Sacrament,
I understand that it is my responsibility to:
Ø
attend
the Meeting for Confirmation Candidates and parents (sponsors also welcome), which is
planned for Sunday, September 11th at 6 PM in
the Parish Hall. Information and forms will be handed out at this meeting. I will
contact the Faith Formation Office if I am unable to attend.
Ø
participate
in the Rite of Enrollment, which publicly
announces the intention of the candidates to prepare for and receive the sacrament of
Confirmation. This will be held at all masses at SMS Church
on September 24th and 25th.
Candidates will receive a blessing at the mass they choose to attend.
Ø
attend
one of the Confirmation preparation classes which will be offered in Sept./Oct.
(TBA).
Ø
complete
and return the Confirmation Information Form to the Faith Formation Office by 10/1/11. This
information will be recorded in the official parish records, and includes selecting a Confirmation
Sponsor and Confirmation name (or choosing to use your baptismal name.)
Ø
turn
in service hours/info to the Faith Formation Office by
10/1/11 for service projects completed during my high school years.
Ø
attend
the Afternoon of Reflection to be held on October
23rd following the 11:45 AM mass.
Ø
attend
mass regularly in order to more fully participate in our Catholic faith.
Ø
read
the *SMS church bulletin weekly to keep informed of announcements and
important information regarding Confirmation.
Ø
contact
the Faith Formation Office to schedule a personal meeting to prepare for Confirmation, if I am
unable to meet the above requirements.
